Required forms

Accepted HCA students must complete the following forms listed below, and other practice requirements, before being permitted to attend lab and/or clinical practice. Completion of the documents can begin once the student has been admitted into the HCA program. Completed documents will be submitted through the School of Nursing’s Moodle site. Students will be notified by the HCA program assistant when submission of pre-requisite documents can be submitted to the Moodle site. It is highly recommended that students have access to a laptop/computer for this program. Assignments are to be submitted electronically, and course information including manuals, schedules, and PowerPoints are accessed via Moodle on the TRU website. There are several computer labs in Old Main that students can access.  

HSPnet consent form

This consent form is used to disclose student information to agencies for arranging practicum placements.

Criminal record consent form

Along with the Criminal Record Consent form, submit two pieces of Photo ID (one of which should be a valid BC Driver’s License, Passport, BC Services Card, or Permanent Resident Card).

Along with the criminal record consent form, submit two pieces of approved ID.

First aid (every two years) and basic life support certificate (yearly)

Accepted courses for first aid are:

  • Standard first aid
  • Emergency first aid
  • Occupational first aid level 1
  • Recertification of previous first aid course
  • First aid courses with theory online and testing in person are acceptable

Basic life support certificate

Certification must take place in an instructor-led, in-person course — not online.

BC Foodsafe Level I certificate

Foodsafe certificates are valid for five years. If our certificate is older than five years, a refresher course will need to be taken prior to lab and/or practice. Check for accepted courses.

School of Nursing immunization form

This immunization form must be completed by a health care provider (public health nurse, physician, pharmacist, travel clinic).

Proof of COVID-19 vaccination

All HCA students are required to be fully vaccinated (two doses, but three is recommended) before going into practica.
